Our falls module supports the provision of  a single point of access (SPA) service, managing referrals, well being  and FRAT assessments.  The system was developed with clinicians and follows the patient pathway from referral to discharge. The system supports clinicians to record their assessments electronically, allowing them to track their patients through the system, whether they are awaiting a well being assessment or part of a 10 week exercise programme.
